LAS VEGAS — After successfully launching an Accountable Care arrangement and population health management partnership last fall, Humana Inc. (NYSE: HUM) and HealthCare Partners Nevada, a division of DaVita HealthCare Partners Inc. (NYSE: DVA), will further solidify the partnership by opening two new primary care practices in Nevada.
Opening under the HealthCare Partners Nevada brand, the two new primary care practices located in Southern Nevada are designed with Humana’s Medicare Advantage members in mind. The new practices are located at 6210 N. Durango Drive, Building 11 in Las Vegas and 745 S. Green Valley Parkway, Suite 160 in Henderson.
The practices will open just in time for the upcoming Medicare open enrollment period that goes from Oct. 15 through Dec. 7 and is when those eligible for Medicare are able to change their health plans and prescription drug coverage for the following year. For many, this is the only time of year these changes can be made.
Last year, HealthCare Partners Nevada, in partnership with Humana, opened three primary care practices in Las Vegas. This was an expansion of Humana’s Integrated Care Delivery Model with DaVita HealthCare Partners.
All HealthCare Partners locations practice the organization’s Total Care Model, an approach to health care in which primary care providers, specialists and professional staff work as a coordinated team to manage all aspects of patients’ care and overall health. This includes a strong focus on prevention and assisting patients with maximizing their health insurance benefits.
“The partnership between HealthCare Partners Nevada and Humana has helped increase access to high-quality, coordinated care for numerous Medicare Advantage patients, and we look forward to reaching even more seniors through these two new clinics,” said Bard Coats, M.D., MBA, market president of HealthCare Partners Nevada. “This expanded partnership reflects the commitment of HealthCare Partners Nevada to provide comprehensive, patient-focused care through our innovative Total Care Model.”
With the two new clinics, HealthCare Partners Nevada now offers a network of more than 310 primary care providers at approximately 100 primary care locations throughout Southern Nevada. The company also provides patients with access to more than 1,700 specialists.
“Partnerships like the one with HealthCare Partners Nevada will help Humana be successful as we venture into our bold goal – to improve the health of the communities we serve 20 percent by 2020 by making it easy for people to achieve their best health,” said Rick Beavin, Nevada market president for Humana’s Senior Products. “HealthCare Partners Nevada’s Total Care Model is already making a difference for our members in Las Vegas, so we are excited that the expansion of this partnership will further provide innovative patient-centered care for our Humana Medicare Advantage members to improve their well-being.”
The Humana and HealthCare Partners Nevada partnership will offer more personalized care and a broad range of population management tools like predictive analytics, chronic disease management and wellness programs.
Through its programs, Humana can provide actionable patient data, providing physicians with a total patient population health picture. The Humana/HealthCare Partners Nevada partnership emphasizes quality outcomes with a focus on care coordination and leverages standard measures defined by the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS), including breast cancer screening, colorectal screening, comprehensive diabetes care, A1c control and high-risk medications.
Today, Humana has approximately 1.5 million individual Medicare Advantage and 200,000 commercial members, cared for by approximately 43,000 primary care physicians in more than 900 accountable care relationships across 43 states and Puerto Rico.
